Unanimously
carried except for Mrs. Firstenberg who abstained.

HEALTH QUEST, ROUTE 9
First item on the agenda was the proposed sign for Health Quest, 1530 Route 9. Todd, a
representative of Health Quest gave his presentation. He stated they had took over the
Medicus site this past June – and one of their goals was to renovate the premises and give
it a face lift. What they are looking at is they wanted to change the name from Medicus
to Health Quest Immediate Care and wanted to change the sign. Mr. Morris asked if
Health Quest were new owners or existing owners just changing the name and Todd
answered they were new owners who took over June 13, 2008. Mr. Morris then asked
Mr. Fenton if the new owners had come in for a new Certificate of Occupancy and Mr.
Fenton answered no they had not. Mr. Morris stated he would address the change of
ownership first. He went on to explain that the reason the Planning Board requests that
new owners come to them is to let them know if there are any current restrictions on their
property and if there are – then it will be written into the record as part of their new
Certificate of Occupancy. It was stated that there would be no changes to the outside of
the building – just cosmetic renovations inside. Mr. McCormick made a motion to
approve the new ownership, seconded by Mr. Moran contingent upon the new owners
filling out the necessary paperwork for a Certificate of Occupancy. Unanimously carried.
Mr. Fenton added that there was a building permit issued when Medicus owned the
building for changing out the mechanicals for new and more efficient units. This
building permit has been closed out by Health Quest.

Next the proposed signs were discussed. Mr. Morris stated it appears that the applicants
are just changing out the existing Medicus signs with Health Quest signs and applicant
answered that is exactly what they are doing. Mr. Morris asked applicants how tall the
road sign is and applicant answered it is approximately 6 ft – in fact he can stand up next
to it – Mr. Morris asked if there were any changes to the height of the sign and Todd
